The transition from a hospital or skilled nursing facility can be confusing. Rehabilitation instructions, prescriptions to fill and follow-up appointments to make are just a few of the many tasks. Having the strength and energy to do all this can be challenging. The process and recovery can be even more overwhelming because one can often be alone during this transition.

Let ADL Homecare help. An ADL Homecare trained caregiver will meet you at the hospital, skilled nursing or other health facility and guide you through the transition process. After your discharge, ADL Homecare will make sure you have what you need. For example, transportation to get you home, schedule follow-up appointments, reminders about prescriptions and medication regimens. We will also schedule a free home safety assessment. ADL Homecare clients will receive the assistance needed to avoid hospital readmission's.
